Specific Course Description:
This
course is designed to assist the individual in his/her responsibility for
establishing good health practices and thereby avoiding illness.
Prerequisite:
None.
Credit Hours:
Two (2) semester hours. Two (2) lecture hours per week.

Course Objectives:
Upon completion of this course the student will be able to:
1. To
explain certain behaviors as either healthy or non-healthy.
2. To
describe, in terms of health behaviors, that which will offer a quality
lifestyle.
3. To
identify behaviors that promote good health.
4. To
describe individual worth of self and others.
5. To
describe life as an event to be enjoyed.
6. To
explain how life can be approached in a positive optimistic manner.
Instructional Mode:
Student
Expectations:
1. Attend class regularly and be on time.
2. Read and prepare assignments prior to class.
Tape recorders may
be used in this class. Cell phones and
beepers are not to be on during this class or lab. Calculators cannot be used during any quiz or
examination.
Student Evaluation:
Evaluation for this course grade will include:
Unit Exams - 30%
Mid-Term Exam - 20%
Final Exam - 20%
Semester Project - 30%
Grading Policy:
A = 90 – 100%
B = 80 – 89%
C = 70 – 79%
D = 60 – 69%
F = 59% or below
Make-up Test
Policy:
A student will be
allowed to make-up the Mid-term and Final examinations. The student will have five days, from the day
the examination was originally given, to make up the examination. Exam must be made up by
Attendance
Policy:
Class attendance
is expected of all students.
Incompletes/Withdrawals:
Incompletes are not assigned unless extenuating circumstances exist.
It is the
student’s responsibility to officially withdraw from a course if the student
decides not to complete the course. Withdrawls
can be completed with a counselor.
Failure of the student to complete the formal withdrawal process will
result in a “failing” grade for the course.
Office Hours:
Office hours will be posted on the instructor’s office door. Additional meeting times can be scheduled individually at mutually convenient hours. Students are encouraged to consult regularly with the instructor regarding class progress or other topics of concern.

COURSE INFORMATION
This course consists of 26 half-hour video programs, the
textbook, workbook and study Guide. In
this packet you will find six different Unit Exams with questions taken from
each respective chapter in the text, the videos and the study guide. Please note the order of the chapters and
lessons to be completed and studied for answers to the questions. You should watch the videos and read the
chapters for maximum learning. You will
receive all the necessary information to answer each question on the Unit Exams
from the text, videos and study guide.
METHODS TO CALCULATE GRADES:
There are
basically three things that you will do to earn credit for the course. They are (1) complete the homework
assignments with the Unit Exams; (2) complete the mid-term and final exams; (3)
complete the Semester Project. A more
detailed description of each requirement follows below.
1. HOMEWORK
ASSIGNMENTS: Six Unit Exams with 100 questions each will represent 30% of your
grade. It is very important to complete
them all and submit them on time. Each
homework assignment will cover specific chapters in the text, video viewing and
study guide lessons. If you hand carry exams
to me, it must be delivered to my office on campus before

PROJECT. The project is worth a maximum
of 100 points. Each student will
complete a semester project for the tele-course. The due date for the semester project is listed
in the class schedule. If you mail the
project to me, it must be postmarked no later than
3. MID-TERM
and FINAL EXAMS: are 100 questions
each. Most (but not all) of the
questions will be taken from the Unit Exams.
Be sure to attend and take the Mid-term and Final Exam at the scheduled
date and time. No exam will be started

HOW DO I GET STARTED?

1. Read this course syllabus from beginning to

2. Read the Introduction Segment of the Study
Guide.

Assignment # 1 and note the due date of the 1st Unit Exam. Pace yourself; you have two weeks to complete
Assignment # 1 and mail to the instructor Exam # 1.

the end of the first week you should have completed your self-assessment in the
workbook and selected your topic for your Semester Project. If you do not understand the assignment after
reading the handout entitled “Semester Project,” please call or e-mail Dr.
Hayduk at the number or address on the front page of the syllabus.

Continue working on your project and the assignments. Once you have mailed in Exams # 1, # 2, and #
3 they will be returned to you to use for studying for the Mid-term. After you mail in Exams # 4, # 5, and # 6
they will be returned so you can use them to study for the final exam.

8. Continue working on your Semester
Project.
Begin to write the “Introduction”
for your Project by
Sept 9th. This means you must
have identified during
Assignment # 1,
your health behavior that you want to change.
It might help you to look at the
possible behaviors listed in
the handout entitled “Semester Project” or to look at the topics
listed in the “Brief Contents” on page M16 at the beginning
of your text. When you select a
topic go to that chapter in
the textbook and read it NOW and complete the appropriate Self-
Assessment for that chapter in the Student Workbook and the Lesson
and Video for that Chapter. By
doing this NOW you should have
enough information to get your Project started. You must include
in your written paper a copy of your initial Self-Assessment from
the workbook and then a second copy of the same Self-Assessment
completed at the end of your Project. When you include these 2
copies of the Self-Assessment in the final paper they should be
identified by the word “Before” and “After”.

your new behavior and documenting data each day.

SEMESTER PROJECT
Purpose
The purpose of this
project will be to identify a negative health behavior that you believe you
possess and would like to do something about it in a positive direction. The project that you do will be for you. Your instructor is the only person who will
read the written portion of your project.
The project is intended to help you recognize your own health behaviors
and determine any behavior that might need some improvement. After recognition of your negative behavior
you will develop a plan to improve your health behavior and then you will
implement the plan over the course of the semester and finally write a paper
about what you accomplished.
Confidentiality of the students identified problem and the content of
the student’s written paper will be maintained.

How To Get Started?
Section # 1 of the Paper -
Deciding to Change
Introduction a. An Introduction which should include general information about the
health-related behavior that you have selected. You should list a minimum of three references
at the end of the paper that you used to obtain information about your
health-related behavior. The textbook,
study guide and student workbook cannot be used as any of the three
references. This section should be at
least 1 ˝ to 2 typed pages doubled space.
The words should be your own do not copy material directly from a
book.
The Problem b. Identify your health-related problem, your
goal for yourself by the end of the semester (make your goal realistic) and the
reason your current health-related behavior is risky.
Reasons c. List the reasons you should make this change
and the benefits for you. Follow the example under # 2 on page 4 of your
Workbook.
Conclusion d. Follow the directions under # 3 on Page 4 of
your Workbook.
Section # 2 of the Paper -
Implementing the Change
a.
There
will be six sections in this part of your paper. Those six sections are listed on pages # 4
and # 5 of your workbook under “Implementing the Change”.
Self-
Assessment b. Make sure you answer all of the questions
under each section. After answering all
the questions put in your paper a copy of the Self-Assessment that you used to
determine you had a problem behavior.
Retake the Self-Assessment at the end of the project and include a copy
of that Self-Assessment, mark them before and after.
Grading of the Project

Your
paper should have a cover page.

The
paper must be typed.
-
The last
page should be a reference list which lists a minimum of three references (The
textbook, workbook, and study guide cannot be counted in the three).

The
paper should be proof-read in order to not have incomplete sentences or
misspelled words.

No
corrections should be made with a pen, pencil or white-out.
-
All
punctuation should be correct.

All
grammar should be correct.
